Hard palate reconstruction with a pedicled osteomyocutaneous mandible flap: case report.

چکیده
Oncologic resection of palatal neoplasms will predictably lead to significant oral dysfunction, affecting both speech intelligibility and deglutition. The resulting velopharyngeal insufficiency may be corrected with the fabrication of an obturator or by flap reconstruction. The use of obturators is associated with a decrease in operating time and elimination of flap donor site morbidity, whether regional or distant. Disadvantages associated with a prosthesis include a prolonged fabrication process of up to 1 year and the high cost, as well as the need for regular removal and cleaning by the patient. Alternatively, flap reconstruction provides for immediate and permanent correction of the oronasal communication, but increases operating room time and is associated with the possibility of donor morbidity at the flap harvest site. The pedicled osteomyocutaneous mandible flap has shown utility in the reconstruction of composite mandibular defects.1 Its use as a method of reconstructing the hard palate represents a novel application of this flap, which is described herein using an illustrative case example.

BACKGROUND The purpose of this study was to present the feasibility of reconstructing an inferior orbital rim and cheek defect with a pedicled osteomyocutaneous submental flap. METHOD A 77-year-old women with a right nasosinusal adenoid cystic carcinoma invading soft tissues and skin of the cheek, the inferior rim of the orbit, the hard palate, and the middle turbinate is presented. A right r...

PURPOSE The purpose of this study was to develop and assess the potential feasibility of reconstructing composite defects of the mandible with a local pedicled osteomyocutaneous mandibular flap. MATERIALS AND METHODS The flap design was established based on anatomic principles. A prospective evaluation of the flap was then performed in a fresh cadaver model, and, subsequently, its vascular in...
متن کاملPalate Mucoperosteum: An Usefull Adjunct in Buccal Mucosa Reconstruction
Background: Palate is a complex structure separating oro- and nasopharynx. However, reconstruction of the defects of palate is much simpler because of the versatile mucoperiosteal flaps. Here, we present our experience of palatal mucoperiosteal flap used in different situations. Methods: Fifteen patients of palatal as well as buccal mucosa defects were reconstructed using either free or pedicl...
متن کاملSubmental Artery Island Flap in Reconstruction of Harde Plate after wide Surgical Resection of Veruccous Carcinoma. Two case reports
Introduction: Reconstruction of intraoral soft tissue defects is important in restoring function and esthetic. In large defects, there will be demand for regional pedicle flaps or free flaps. Hard palate separates nasal and oral cavities. Due to the small surface area between flap and remaining palate after surgical resections, optimal blood supply of the flaps for hard palate reconstructions ...

BACKGROUND With the idea of "replacing lost tissure with similar tissure in kind", vessel-pedicled palate mucosal flap, pedicled buccal musculomucosal flap and adjacent tongue musculomucosal flap could be the ideal approaches to soft palate reconstruction. AIM To assess the adjacent tongue musculomucosal flap for soft palate reconstruction. PATIENTS AND METHODS From August 2010 to July 2011...
